Revision Eyelid Surgery – JW Plastic Surgery Hospital
Addressing Imperfections, Restoring Confidence
The eyes, often considered the windows to the soul, are a prominent facial feature. When eyelid surgery doesn’t yield the desired results, it can be disheartening. At JW Hospital, our experienced plastic surgeons specialize in revision eyelid surgery, correcting imperfections and restoring a natural, beautiful appearance.
Understanding Revision Eyelid Surgery
Revision eyelid surgery is designed to rectify issues that may arise from previous procedures, such as:
- Unnatural Appearance: Uneven or overly pronounced eyelid folds.
- Scarring: Visible scars from prior surgery.
- Sagging: Drooping eyelids or under-eye bags.
- Functional Issues: Difficulty closing the eyes completely or excessive tearing.

What To Expect
- Surgery Duration: Depends on the type
- Hospitalization: Not Required
- Number of Postoperative visits: 2-3 times
- Anesthesia: Short-term sedation or local anesthesia
- Removal of Stitches: 7 days after the operation
- Recovery Period: Removal of stitches on 7th day
Why Consider Revision Eyelid Surgery
Revision eyelid surgery may be recommended for:
- Thick Double Eyelids: When the double eyelid fold appears excessively thick, creating a “sausage” effect.
- Visible Scars: If previous eyelid surgery or injury has left noticeable scars.
- Untied Double Eyelids: When the double eyelid crease is inconsistent or disappears intermittently.
- Thin Eyelids: If the double eyelid fold is extremely narrow or the eyelid skin appears excessively thin.
Unsatisfying Eye Shape: When the overall shape or appearance of the eyes does not meet the individual’s desired aesthetic goals.
Our Approach to Revision Eyelid Surgery
Our surgeons carefully assess each patient’s individual needs and develop a personalized treatment plan. Common revision procedures include:
1. Revision Lower Blepharoplasty
If the initial lower blepharoplasty only involved fat removal or repositioning, the skin under the eyes may sag over time, even if the initial results seemed satisfactory.
2. Revision Canthoplasty
If you are dissatisfied with the results of a previous epicanthoplasty or lateral canthoplasty, or if the lacrimal lakes (the triangular spaces where tears collect) are excessively exposed, revision canthoplasty can correct these issues.
Revision canthoplasty
Steps:
- Step 01: Create a V-Y advancement flap on the inner canthus.
- Step 02: Close the V-Y advancement flap.
- Step 03: Design a Z-plasty.
- Step 04: Perform the Z-plasty.
Tip: Why should a Z-plasty be performed?
- A Z-plasty helps prevent the inner canthus from transforming into an undesirable U-shape, a potential complication of revision canthoplasty using a triangular flap. Z-plasty contributes to a more aesthetically pleasing and natural-looking inner canthus.
3. Revision Lateral Canthoplasty
If you are experiencing issues such as excessive mucosal exposure, lower eyelid displacement, ectropion (outward turning of the lower eyelid), or an altered eye shape resembling “webbed feet,” revision lateral canthoplasty can address these concerns.
Revision Lateral Canthoplasty
Steps:
- Step 01: Make an incision along the scar of the previous lateral canthoplasty and the exposed mucosa.
- Step 02: If ectropion is present due to lower eyelid displacement, perform aesthetic canthal suspension surgery to reposition the outer corners of the eyes back to their original location.
- Step 03: Close the incision layer-by-layer, carefully considering the original structure of the outer corners of the eyes.
